An international research team led by the University of Milan in Italy discovered the gene sequence of COVID from a biopsy sample of a young female patient with skin disease in the country on Nov 10, 2019, according to Italian media outlet ANSA and news website the Republic. This result has moved the appearance of "patient zero" in Italy to November 2019.

The results were published in the British Journal of Dermatology. The researchers tested biopsy samples from a patient with atypical skin disease who had not been diagnosed in the last few months of 2019 and found the patient was infected with COVID. Her symptoms were only red patches on her skin and a mild sore throat. In addition, the patient was positive for antibodies to COVID in a serum test taken in June. Researchers from the university believe this is the earliest coronavirus infection found in Italy.

Janotti, from the University of Milan and leader of the research team, told media in the next study, new coronavirus infection samples may be found from earlier than November 2019. He said the study found some cases showed only skin symptoms, so they wanted to know whether any skin disease patients before the outbreak had been infected.

Previously, researchers had detected the coronavirus in municipal wastewater samples from the northern cities of Milan and Turin in December 2019. In November 2020, researchers from the National Institute of oncology and other institutions in Milan, published a paper in the National Journal of oncology, saying that the coronavirus may have spread in Italy as early as September 2019, a few months earlier than the first confirmed case of COVID reported in Italy.
